Fatalities

Work-related fatalities are deaths that occur as a result of injury from work. This data includes workers who die from injuries while working and members of the public who die as a result of someone else’s work activity. It excludes deaths from natural causes and self-harm. The data is unable to include deaths from occupational disease (see work-related health). This is a single work-related fatalities dataset, combining notifications to WorkSafe and data from other agencies. It provides a whole-of-system picture of New Zealand’s health and safety performance. Note: Reporting is paused while we update our system. Information may be requested under the Official Information Act during this pause: www.worksafe.govt.nz/contact-us/oia-request/

Notifications made to WorkSafe
Concerns

Notifications of Concern and H&S Concern about unsafe conditions include complaints about unsafe work practices or notifications about potentially unsafe conditions.

Incidents

A notifiable incident is one where someone has been exposed to a serious and immediate risk of harm.

Injury, illness and serious harm

The Health and Safety at Work Act (2015) (HSWA) requires that we are notified if someone becomes seriously ill or is seriously injured as a result of work. Notifying allows us to immediately investigate or follow up on the events that caused the injury or illness.

WorkSafe activities
Enforcement activities

Enforcement activities are those that result from our assessments and investigations but exclude prosecutions.

Investigations

We investigate potential breaches of the Health and Safety at Work Act 2015. Note: Reporting is paused while we update our system. Information may be requested under the Official Information Act during this pause: www.worksafe.govt.nz/contact-us/oia-request/

Assessments

WorkSafe assessments involve our staff visiting workplaces around New Zealand. Inspections of high hazard facilities are also included.
